Before a euthanasia appointment can take place, we ask that a consent form is completed. This allows us, and the attending vet, to:

• Confirm your permission for the procedure to go ahead
• Ensure the correct pet and owner details are recorded
• Make sure everything is handled in line with your preferences

It also gives you the opportunity to take a moment, review your decisions, and feel confident in the choices you’ve made.

We politely request that the consent form is completed in advance of your appointment.This helps to avoid any unnecessary stress on the day and allows you to focus fully on being with your pet. If this isn’t possible, the form can also be completed at the time of the visit.
