Why We Created This Questionnaire
As pet lovers ourselves, we know that no one wants to see their pet suffer. But it can be difficult to tell whether your pet is still enjoying life – especially when emotions are high.
Our online questionnaire is based on the well-established Five Freedoms model, a trusted veterinary framework that looks at the following key areas:
- Pain levels – Is your pet comfortable or showing signs of distress?
- Appetite & hydration – Are they eating, drinking, and maintaining weight?
- Mobility & hygiene – Can they move freely, groom themselves, and go to the toilet comfortably?
- Mental wellbeing – Do they still enjoy affection, toys, or their normal routine?
- Good days vs. bad days – Are the happy moments still outweighing the difficult ones?
